Medical Records Requests
We will provide you or another physician or health care provider a copy of the medical records generated by our practice at your request.
Information from other physician's offices, such as consult notes and referral reports, cannot be released by our office and should be requested directly from the physician's office that created those records.
Since our records are created and maintained electronically, we generally release them in electronic format, either burned to a disc or by fax, for a minimal processing fee.
There is no charge for records requested directly from another physician's office or hospital.
If you prefer, we can print hard copies of your records for 25 cents per page in addition to the $10 processing fee.
