Paperwork and Form Completion
We try to complete paperwork needed to help with insurance, employment or other purposes as quickly as we can. But please understand that each of our physicians see dozens of these forms every week, they are generally very different from each other and take time to complete.
Additionally, we are busy seeing patients throughout the workday and week, so forms are typically completed after hours, over lunch or before seeing patients.
Though we can often complete forms sooner, please try to give us at least a week to complete routine forms.
