If you are transferring care from another provider, please bring copies of pertinent medical records from that provider with you to your first appointment.  Or, have those records faxed directly to our clinic at 806.354.5626, prior to your first appointment. 

We are legally bound to protect the confidentiality of your medical record, so nothing can be released from our clinic without a signed release form.  These can be obtained at our front desk.
